You’ll be sent links to complete the following before your appointment:
Adult ADHD Self-Report (ASRS)
ACE+IR Questionnaire
Medical & Contact Info form
Record Sharing Consent form
Please complete these at least 2 days before your appointment.
-
Let us know as soon as possible. We may need to reschedule your appointment to make sure your clinician has everything they need.
-
Yes — we just ask for at least 48 hours’ notice for any changes or charges may apply.
-
The clinician will complete a structured diagnostic interview using the DIVA 5. This explores your experiences of attention, hyperactivity, and impulsivity during childhood and adulthood. It takes around 60- 90 minutes.
-
In most cases, yes. If the clinician needs further information — such as input from a parent, teacher, or GP — they’ll let you know what’s needed and what to expect next.
-
You’ll receive a written summary within 10-14 days. This includes your diagnosis (if confirmed), plus a personalised care plan which may include ADHD coaching, therapy, lifestyle support, or medication.
-
That’s absolutely fine. If you decide not to explore medication, we’ll still offer personalised recommendations tailored to your needs. This might include ADHD coaching, workplace adjustments, lifestyle strategies, or referrals for talking therapy or other services.
-
If medication is appropriate and something you’re open to, your clinician will talk you through the options, what to expect, and how the process works. Before prescribing, we’ll need up-to-date blood pressure and heart rate readings. Once confirmed, we’ll begin the titration phase.
-
Once your prescription is approved, it will be issued using SignatureRx – a secure digital prescribing platform used by private healthcare providers.
Your prescription will be sent directly to one named pharmacy, either:
Chosen by you (if you provide a preferred pharmacy in advance), or
Selected by us (typically a pharmacy near your address or Signature Pharmacy, our trusted delivery partner)
Once a prescription has been issued, the pharmacy cannot be changed, so it’s important to confirm your preference early.
You’ll receive a confirmation once the prescription has been sent, along with instructions for collection or delivery.
Option 1: Local Pharmacy Collection
If you’d like to collect your medication in person, we can send your prescription to a local pharmacy that accepts controlled private prescriptions.
Please provide the pharmacy name and postcode when we confirm your prescription plan.
Note: Prescriptions issued via SignatureRx are locked to one named pharmacy and cannot be transferred elsewhere once sent.
Option 2: Home Delivery (via Signature Pharmacy)
If no preferred pharmacy is given, we will use Signature Pharmacy (trading name of Signature Healthcare Services Limited), who provide:
Tracked next-day UK delivery in discreet packaging
Free standard delivery
A reliable service widely used by ADHD clinics across the UK
Before We Issue Your Prescription
To avoid delays, please confirm:
Your preferred pharmacy name and postcode, if applicable
Whether you would like to use home delivery via Signature Pharmacy
If no preference is given, we will default to Signature Pharmacy for convenience and safety.

Titration is the process of safely finding the most effective ADHD medication and dose for you. You’ll start on a low dose, with weekly check-ins to monitor symptoms, side effects, and progress. The process usually takes around 6–8 weeks.
-
Once a stable dose has been found (or if you decide not to continue), you’ll move on to regular review appointments — typically every 3 to 6 months. These are a chance to check in, adjust your plan if needed, and make sure you’re getting the right support.
