We welcome referrals for both children and adults from GPs, educational professionals, and other healthcare providers who are supporting individuals with possible ADHD, autism, or related mental health concerns.
To help us offer timely and appropriate support, please complete the secure referral form below with as much relevant detail as possible.
You can upload supporting documents such as medical letters, school reports, or previous assessments.
Please ensure the patient, or their parent or guardian in the case of a child - has given informed consent before making a referral.
All referrals are reviewed by our clinical team and triaged according to clinical need.

What Happens Next?
Initial Review
After you submit the form, our experienced clinical team will carefully review the information to understand the individual’s needs and determine whether an assessment is clinically appropriate.
Triage & Contact
If the referral is suitable, we’ll reach out to the individual (or their parent/carer) to gather any further information and discuss the most appropriate next steps based on the concerns raised.
Assessment Booking
Where appropriate, we’ll offer an appointment for an ADHD, autism, or general mental health assessment. All assessments are carried out by qualified clinicians who specialise in neurodevelopmental and mental health care.
Follow-up Support
Following assessment, we’ll guide the individual through any recommended support — which may include ADHD medication titration, autism support planning, coaching, or a referral to another service if needed.

Our clinicians follow evidence-based practice in line with NICE guidelines, including ADHD (NG87) and autism (CG142/CG170). All medical professionals are registered with the GMC. Children are assessed by clinicians with specialist experience in child and adolescent mental health (CAMHS). We are committed to the highest standards of clinical governance and patient safety.

This policy outlines how Cognition Care manages referrals for ADHD, autism, and other mental health concerns in children and adults, ensuring safety, clinical appropriateness, and compliance with CQC standards

We accept referrals from:
GPs and medical professionals
CAMHS and NHS services
Educational professionals (e.g. SENCOs)
Allied health professionals
Parents/carers or self-referring adults (where appropriate)

Referrals can be made using our secure online form. You’ll be asked to provide details about the patient, your professional role, reason for referral, and upload any supporting documents.

To process a referral, we ask for:
Referrer and patient contact information
Reason for referral and relevant history
Any supporting documents (school reports, letters, etc.)
Confirmation that informed consent has been obtained

Please ensure the patient, or their parent/guardian if under 18, has given informed consent. All information is processed in accordance with GDPR and our Privacy Policy.

Each referral is:
Reviewed by a clinician
Triaged according to clinical need
Followed up with a phone call or email to arrange next steps
If a referral is unsuitable, we will explain why and suggest alternatives.

If any safeguarding concerns are raised in a referral, these are flagged immediately and followed up in line with our Safeguarding Policy.

This referral policy is reviewed annually or sooner if regulations change.
