Patient Terms & Conditions

  • Cognition Care is a CQC-registered, UK-based independent mental health service specialising in ADHD, autism, and general psychiatry. We offer assessments, treatment, and coaching services tailored to neurodivergent individuals and those seeking support with mental health.

  • All clinicians at Cognition Care are fully qualified and registered with the appropriate UK regulatory bodies (e.g. GMC, NMC). We ensure all professionals adhere to the latest NICE guidelines and maintain appropriate indemnity insurance.

  • Appointments are delivered online via secure video call unless otherwise stated. It is your responsibility to ensure you have a suitable device and internet connection. If you are more than 15 minutes late, we may need to reschedule and you may still be charged.

  • We require at least 48 hours’ notice for cancellations or rescheduling. If you cancel with less than 48 hours’ notice or do not attend, the full fee will be charged. This allows us to offer the slot to others on our waiting list.

  • All fees are clearly stated on our website.

    • Clinical appointments (e.g. assessments, titration, reviews) require full payment at the time of booking unless otherwise agreed. For ongoing treatment, such as titration, a payment schedule may be arranged in advance.

    • ADHD coaching sessions are billed separately from clinical services and are not eligible for insurance reimbursement. Payment is required in advance of each session or in blocks, if a package has been agreed.

    We do not currently offer NHS-funded appointments. Any changes to fees will be communicated in advance.

  • Prescriptions are provided at the clinician’s discretion following a clinical assessment. We use SignatureRx to send private prescriptions electronically to a UK pharmacy of your choice.

    Please note:

    • You are responsible for paying the pharmacy directly for any medication prescribed. This cost is separate from your appointment fee.

    • Controlled drugs (e.g. ADHD medication) will only be prescribed once appropriate monitoring (e.g. blood pressure, heart rate) has been completed and is up to date.

    Prescription renewals and ongoing medication reviews may incur additional fees, which will be clearly communicated in advance.

  • If you begin ADHD medication with us, you will enter a titration phase involving regular check-ins to ensure safety and effectiveness. You are expected to complete monitoring forms and provide health data as requested. Medication may be paused if data is missing or concerning.

  • Cognition Care is committed to protecting your privacy and confidentiality in line with the UK GDPR, the Data Protection Act 2018, and Care Quality Commission requirements.

    • Your information is stored securely on Semble, our GDPR-compliant clinical system, and is only accessed by authorised staff involved in your care.

    • All staff receive regular training on data protection and confidentiality.

    • We will never share your personal data without your explicit consent unless there is a legal requirement, a serious risk of harm, or a safeguarding concern.

    You have the right to request access to your records, request corrections, or raise concerns about how your data is handled.

  • Cognition Care has a duty of care to protect all patients, especially those who may be vulnerable, and complies fully with CQC safeguarding standards and the Children Act 1989 / Care Act 2014.

    • If we are concerned that you, a child, or someone else is at risk of harm, abuse, or neglect, we may need to share relevant information with appropriate safeguarding agencies (e.g. local authority, GP, emergency services).

    • We will always aim to discuss any concerns with you before taking action, unless doing so would increase risk.

    • All clinicians and staff receive regular safeguarding training and follow Cognition Care’s internal safeguarding policy, which includes clear reporting procedures and named safeguarding leads.

  • We are committed to providing high-quality care and welcome feedback to help us improve. If you are unhappy with any part of your experience at Cognition Care, we encourage you to raise it with us.

    🟡 Our complaints process:

    1. Informal Resolution
      You can raise concerns directly with our admin team or clinician during or after your appointment. We aim to resolve issues quickly and informally wherever possible.

    2. Formal Complaint
      If you prefer to raise a formal complaint, please email us at admin@cognitioncare.uk with the details. We are happy to assist if you need support writing your complaint.

    3. Acknowledgement and Investigation
      We will acknowledge your complaint within 3 working days and aim to investigate and respond within 20 working days. If more time is required due to complexity, we will keep you updated throughout.

    4. Response and Outcome
      We will send a written response outlining our findings and any actions taken. If you are not satisfied with the outcome, you may escalate your complaint externally.

  • By booking and attending an appointment at Cognition Care, you confirm that:

    • You understand the nature and purpose of the assessment, treatment, or service being offered

    • You give your informed and voluntary consent to proceed

    • You understand that you may withdraw consent at any time without it affecting your right to care or support

    Consent is recorded during your initial registration and may be reviewed or updated during your care journey. If a situation arises where capacity to consent is in question, we will follow the principles of the Mental Capacity Act 2005.

    Our admin team will respond to queries as promptly as possible, usually within 2 working days.

    Please note: Cognition Care is not an emergency service.
    If you or someone else is at immediate risk of harm, please contact 999 or attend your nearest A&E.
    For urgent mental health support, contact your GP or local NHS crisis team.

  • Cognition Care is committed to inclusive, person-centred care.
    If you require any reasonable adjustments to support your access to our services — such as additional appointment time, communication support, or help completing forms — please let us know.

    We aim to make our services accessible to all clients, including those who are neurodivergent, disabled, or have additional needs.

  • Your information is only shared with other healthcare professionals (such as your GP) with your written consent, unless there is a safeguarding concern or legal requirement.

    Before your appointment, you will be asked to complete a Record Sharing Consent Form to indicate your preferences. You can change your consent at any time.

  • By booking with Cognition Care, you agree to the following financial terms:

    • All fees are clearly stated on our website or in your service agreement.

    • Payments must be made in advance unless agreed otherwise.

    • Fees are non-refundable if appointments are cancelled with less than 48 hours’ notice or missed without prior notification.

    • Additional charges may apply for:

      • Prescription reissues

      • Specialist reports or letters

      • Follow-up appointments or medication reviews outside standard packages

    • We do not currently accept private health insurance or NHS funding.

    • Payment is accepted via our secure online booking system or approved invoice.

    If you believe you have been charged in error, please contact us as soon as possible so we can investigate and resolve the issue.

  • By booking and attending an appointment with Cognition Care, you confirm that you have read, understood, and agreed to these Patient Terms and Conditions.

    These terms may be updated periodically to reflect changes in service delivery, clinical guidance, or regulatory requirements. The most current version will always be available on our website.

  • Cognition Care is committed to ensuring that all patients feel safe, respected, and supported during their care.
    You are welcome to request a chaperone during any consultation - whether online or in-person.

    A chaperone may be a clinical team member or a trusted companion of your choice.
    We will always offer a chaperone for appointments involving physical assessments or when discussing sensitive matters, especially with children or vulnerable adults.

    If you would like a chaperone present at any time, please let us know in advance or speak to your clinician during the appointment.

  • We are committed to obtaining informed consent before providing any treatment, assessment, or intervention.

    We will always:

    • Explain the nature, purpose, risks, and benefits of any service or recommendation

    • Offer time to consider your options and ask questions

    • Respect your right to decline or withdraw consent at any point

    For children or individuals who may lack capacity to consent, we will seek consent from a parent, guardian, or legally authorised representative.
    We follow the Mental Capacity Act (2005) in assessing capacity and supporting patients to make informed decisions wherever possible.
